PSSA/NSSA member clubs: 

 

Please forward all Referee applications and moneys to PSSA Director Tom Wolfe for credentialing and signing.  Tom will then forward all to NSSA, checks should be made out to NSSA.  The club’s examining officer should sign and date the form after applicant successfully completes the current Referee exam.
